Direct Deposit Authorization

I authorize Ellement Consulting Group (“Ellement”) to deposit payment for my processed claims directly into my bank account using the details I have provided. This authorization will remain in effect until I update or terminate my banking information through the Member Portal, or until I provide written notice to Ellement in sufficient time to allow for processing.

The banking information provided will be used solely to facilitate electronic funds transfer (EFT) payments for eligible claims and payments administered by Ellement. I understand that Ellement may, without prior notice, stop direct deposit and issue payments by cheque instead. I also understand that if an overpayment is made in error, Ellement may correct it by debiting my account for the amount of the overpayment.

If the banking information provided does not belong to you, you acknowledge and agree that Ellement shall not be liable for any deposits made to that account, and that you shall be solely responsible for any consequences arising from funds being directed to an account not under your name.

By accepting this authorization, I agree that it has the same effect as providing my signature on a paper form.
